Volleyball Camp Registration Form

Join us June 16-18, 2014 for the 4th Annual Indian Hills Warrior Volleyball Camp. This camp will teach athletes all the basic fundamentals of volleyball: passing, setting, hitting, blocking, and serving. The IHCC Volleyball Camp offers a solid foundation in skill development and motivation for your athletes!

The three-day camp will be held Monday, June 16 - Wednesday, June 18 at the Tom Arnold Net Center, Indian Hills Main Campus, Ottumwa.

Camp sessions are as follows:

Grades 4-6: 9:00 to 10:30 a.m. Cost: $35

Grades 7-8: 10:30 a.m. to 12:30 p.m. Cost: $45

Registration must be completed by June 6 to guarantee your camp t-shirt.

Payment: Registration fee can be made by cash or check at the Indian Hills Athletics Office, 608 Indian Hills Drive, Ottumwa, IA 52501. Camp t-shirt is included.

    Upon submission of this form, I hereby authorize the coaches of Indian Hills Community College Volleyball Camp to act accordingly to their best judgment in any emergency requiring medical attention. A recent physical examination for my daughter indicates no reason she should not participate in the activities of the camp.

    As parent/guardian of the participant in the volleyball camp, I recognize and acknowledge that there are certain risks of physical injury and I agree to assume full risk of any injury, including death, damages or loss which the participant may sustain as a result of participating in, and all activities connected with or associated with Indian Hills Community College.

    As parent/guardian, I do hereby fully release and discharge Indian Hills Community College and its officers, agents, and employees from any and all claims from injuries, including death, damages or loss which the participant may sustain arising out of connection with, or in any way associated with Indian Hills Community College.
